U.S. Passports

The Plainfield Charter Township Clerk’s office has been a Passport Acceptance Agency for the U.S. Department of State since September 19, 2001. Our hours of acceptance are 9:00 AM until 4:00 PM, Monday through Friday. Every effort is made to keep the office open during these hours; however, there are occasions when the office must be closed. It would be wise to call (616) 364-8466 to confirm that the Passport Agency is open on the day you wish to come.

US Electronic Passport: Since August 2007, the US has been issuing only e-passports. The US Electronic Passport (e-passport) is the same as a regular passport with the addition of a small contactless integrated circuit (computer chip) embedded in the back cover. The chip securely stores the same data visually displayed on the photo page of the passport, and additionally includes a digital photograph. The inclusion of the digital photograph enables biometric comparison, through the use of facial recognition technology, at international borders. The U.S. e-passport also has a new look, incorporating additional anti-fraud and security features. Passports without chips will still be valid for the full extent of their validity period.

Print Passport Application Forms: The following forms and application can be reviewed, completed and printed at www.travel.state.gov.

  • Application for Passport: DS-11
  • Application for Passport by Mail (Renewals Only): DS-82
  • Application for Additional Visa Pages: DS-4085
  • US Passport Re-Application Form: DS-5504 (Changes/Corrections to a Current Valid Passport)
  • Statement Regarding Lost or Stolen Passport: DS-64
  • Statement of Consent: Issuing a Passport to a Minor Under Age 16: DS-3053

Guidelines for Downloading and Printing Documents: The forms you print must be clear and sharp. Applications that are blurred or illegible will not be accepted. They must be printed in black print on white paper. The paper must be 8 1/2 inches by 11 inches, with no holes or perforations, at least medium (20 lb.) weight, and with a matte surface. Thermal paper, dye-sublimation paper, special inkjet paper, and other shiny papers are not acceptable.

Some printers will reduce the photograph area on the passport application to less than 2x2 inches. Please make sure that the photograph area on the printed application form is 2x2 inches to ensure that the photographs you submit (also 2x2 inches in size) can be correctly attached

Please type or print legibly when completing all sections of the application.

When you apply for a passport, please bring the following information:

  • Proof of identity (state driver’s license, state identification card or US Military ID)
  • Proof of citizenship (Certified birth certificate - not a birth registration or hospital certificate).
  • A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar’s signature, and the date the certificate was filed with the registrar's office, which must be within one (1) year of your birth. Please note that some short (abstract) versions of birth certificates may not be acceptable for passport purposes. Provide a Social Security Number. If you do not provide your Social Security Number, the Internal Revenue Service may impose a $500 penalty.
  • Two passport photos. Passport photos taken at the Township Offices will cost $10.00 per set.
  • Passport photo specification:
    •  2x2 inches in size
    •  Taken within the past 6 months, showing current appearance
    •  Identical
    •  Color
    •  Full face, front view with a plain white or off-white background
    •  Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
    •  Taken in normal street attire - Uniforms should not be worn in photographs except religious attire that is worn daily.
    •  Do not wear a hat or headgear that obscures the hair or hairline.
    •  If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture.
    •  Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons. A medical certificate may be required.
  • Each person must appear in person. There are no exceptions, no matter what age. Both parents or legal guardians must present evidence of identity, in person when they apply for a minor under the age of 16.
  • Two checks: One made out the U.S. Department of State and one to Plainfield Charter Township.

Each applicant must come to the office to complete the Passport Application form and sign it in person. There are no exceptions. There are special rules for children, expedited applications, and unusual circumstances. Please or call to ask your questions before coming to the office. It takes up to six weeks to get a passport, so early planning is a good idea. The Clerk’s office staff is happy to assist you with your application and your questions; call us at (616) 364-8466.

Special Note: US citizens may begin applying in advance for the new US Passport Card beginning February 01, 2008, in anticipation of land border travel document requirements. The Department of State expects cards will be available and mailed to applicants in spring 2008.

The passport card will facilitate entry and expedite document processing at US land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. It will otherwise carry the rights and privileges of the US passport book and will be adjudicated to the exact same standards.

The Department of State is issuing this passport card in response to the needs of border resident communities for a less expensive and more portable alternative to the traditional passport book. The card will have the same validity period as a passport book: 10 years for an adult, five for children 15 and younger. For adults who already have a passport book, they may apply for the card as a passport renewal and pay only $20. First-time applicants will pay $45 for adult cards and $35 for children.

To facilitate the frequent travel of US citizens living in border communities and to meet Department of Homeland Securities operational needs at land borders, the passport card will contain a vicinity-read radio frequency identification (RFID) chip. This chip will link the card to a stored record in secure government databases. There will be no personal information written to the RFID chip itself.

NO credit cards or cash will be accepted, as all documents are mailed to a central passport-processing agency.

Checking the Status of your Passport Application: www.travel.state.gov

To check the status of your application online you will need:

  • Your last name, including suffixes without punctuations except the hyphen, (for example: Jones III, Patton Jr, Jackson-Smith, O Brien, Mc Queen, Varela Garcia)
  • Date of birth in the following format: MM/DD/YYYY
  • The last four digits of your Social Security Number.

Currently, it is taking 7-10 days for passport applications to be tracked online. Please allow at least that time before checking the status of your application.

Expedite Service - Mail: If you apply for your passport today and request Expedited Passport Processing, you will be charged an additional $60.00 per passport application, payable to the US Department of State and can expect to receive your passport(s) in about 3 weeks. The 3-week estimated timeframe for receipt of your passport takes into consideration the additional time it may take to receive your passport application at a Passport Agency/Center when you apply for Expedited Passport Processing through a designated Passport Acceptance Facility and for the passport to be mailed to you. It may take longer for customers who apply for a renewal by mail without "Expedite" marked on the mailing envelope. Effective August 16, 2007, the Department’s standard for expedited passport processing completion, as defined in 22 CFR 51.66, is ten business days from either: 1) receipt of your passport application at a Passport Agency/Center; or 2) if the passport application is already at a Passport Agency/Center, the date your request for expedited passport processing is approved. Expedited passport processing is considered complete when it is put in the mail to the applicant.

Passport demand is seasonal in nature with peak periods falling between January and July. We recommend applying outside these times whenever possible.

Expedite Service - In-Person: If you need your passport urgently, generally in less than 2 weeks, you may contact the Chicago Passport Agency directly. The Chicago Passport Agency takes applications only by appointment. Please call in advance.

THERE IS NO CHARGE FOR AN APPOINTMENT AT A PASSPORT AGENCY. CUSTOMERS SHOULD NOT PAY ANYONE OR ANY BUSINESS MAKING SUCH A CHARGE.

To maximize customer comfort and comply with fire and safety regulations, Passport Agency public waiting areas are limited to applicants, required witnesses, and those accompanying minors, disabled applicants, or serving as interpreters.
